Two cases of pyothorax-associated lymphoma

Abstract

|
Pyothorax-associated lymphoma (PAL) are rare and closely associated with long-standing pyothorax. Most cases have a clinical history of artificaial pneumothorax for the treatment of pulmonary tuberculosis or tuberculous pleuritis. Although the definitive mechanisms of oncogenesis are not yet resolved, geographical, environmental and/or genetic factors are assumed to be involved because most of the cases have been reported in Japan. Recently, several investigators observed EBV gene expression in these types of lymphoma cells, implying a relationship between EBV infection and the lymphoma genesis of PALs. In this report, we present two cases of PAL. One case occurred after therapeutic pneumothorax because of tuberculous pleuritis and were characterized by a pleural mass extending to the thoracic wall, which on histological examination were seen to consist of large elements with immunoblastic morphology. The other case occurred after long-standing tuberculous pyothorax. There was evidence of the lymphoma permeation in bone marrow. Immunohistochemistry showed the expression of LCA and CD20 in all cases.
